Pathway to Human Thriving

The Pathway to Human Thriving is an assessment tool that helps guide clients toward stability across eight important dimensions.

The tool supports individuals experiencing homelessness through assessing their current needs, tracking their progress, and helping to develop achievable goals and next steps.

The Pathway to Human Thriving contains two key components:

  1. An array to understand important human needs

  2. A scoring system for each of these

Individualized Care Plan

After completing the Pathway to Human Thriving assessment, case managers meet with each client to create an Individualized Care Plan. An Individualized Care Plan is a list of short- and long-term goals that case managers and clients set together.

These goals are focused on the Pathway to Human Thriving assessment categories that the client would most like to focus on.

As each client makes progress on their goals, the case manager updates the Pathway to Human Thriving scores and sets new goals with the client.
